Fill out our "New Client Enrollment Form" with all your demographic and insurance information. Be sure to let us know your availability and counseling preferences.

A member of our intake team will contact you within 24 - 48 business hours. This is why it is imperative that you complete the entire form to receive your preferences as close as possible.
03.
Prepare for your first appointment by registering for your portal account, filling out your paperwork. We will answer any questions you may have. Create a list of questions to ask if you prefer.
